Patient Registration
If you wish to register yourself or any family member, please call into the surgery or complete the pre-registration form registration form by clicking on the link at the bottom this page. The form must be signed by the person wishing to register unless under 16 years of age.
When you have completed the registration form your notes will then be requested from your old doctors. The notes are first returned to the health authority and this can take around 8 weeks.
Finally you will be asked to book a new patient check with the practice nurse
Pre-Registration with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. You will not be registered at the practice until the registration form is signed.
